To ensure compliance with IRS regulations regarding the deadline by which Wage and Tax Statements must be issued, all 2007 W-2 Forms are sent from the corporate office via U.S. Mail to the address that appeared on your last paycheck of 2007.

To request a duplicate, provide the following information.  If you have access to a printer, you can simply type that information in the squares below and print it.  Otherwise LEGIBLY provide the same four items on a sheet of paper:

Filing a change of address AFTER the final pay cycle of 2007 has been processed  will not correct the address to which your 2007 W-2 will be sent.  Unless you have a valid forwarding order on file at the Post Office, you will need to request a duplicate.  (This is but one of many reasons why it is important that you always keep your address current.)

Bubba does not carry copies of W-2 forms to gigs.  Calling or visiting the venue  where you worked is not a means by which you may obtain a duplicate; however, Bubba may be able to assist you in completing the steps necessary if you have no internet access.

Requests for duplicate W-2 forms will be accepted ONLY by U.S. Mail; requests made via phone, fax, email or in person will not be accepted.  Duplicates will be sent out ONLY by U.S. Mail; requests to supply duplicates via fax, email, courier or in person will not be honored.  Valid requests will be honored as quickly as possible.  Also, you always have the option of obtaining your W-2 copy directly from the IRS by requesting a transcript, but those are not available until nine months after the tax year ends, so it would be necessary to file an extension.  Visit their website for details.

A duplicate W-2 form may only be requested by the person to which it was issued - not by an accountant, tax preparation company representative, relative, etc., - because the request will be subjected to signature verification prior to processing as an identity security measure.
